Mark J. Sontag, MD, is a Physiatrist who enjoys an excellent reputation in pain management. He is Board Certified in Physical Medicine and Rehabilitation, Electrodiagnostic Medicine and is a QME. His expertise includes spine, sports, cumulative trauma and repetitive strain injures, orthopedic and neurological disorders and pain management. He directed Sequoia Hospitals Occupational Medical Clinic and the In Patient Rehabilitation Unit. He is a team physician for the Oakland Raiders, San Jose Sharks and San Jose Sabercats. He has experience treating chronic pain in the functional restoration setting and has published book chapters and articles on chronic pain treatment.

David Smolins, MD, is a Board Certified Anesthesiologist with a specialization in pain management from the University of California, San Francisco. His training includes epidural steroid injections, sympathetic blocks, spinal cord stimulators, discography, and radio frequency ablation he employs in his private practice. For treatment of chronic pain he emphasizes a functional restoration approach for treatment chronic pain emphasizing interdisciplinary care.

Elaine S. Date, MD, graduated from The Medical College of Pennsylvania Medial School, and Stanford Hospital and Clinics Physical Medicine & Rehabilitation Residency Program. She is certified in Electrodiagnostic Medicine, Medical Acupuncture from UCLA, and Board-certified in Physical Medicine & Rehabilitation. She served for over 20-years as an Associate Professor on the medical faculty at Stanford University School of Medicine. She has published numerous scientific articles and frequently speaks on pain management. She served as a team physician for Stanford women’s volleyball and gymnastics teams.

Diane Hafner, DC, is a licensed Chiropractor specializing in chronic pain-physical rehabilitation. She is a graduate of University of California Berkeley. She provides exercise therapy with yoga resposturizing stretches to improve range of motion, flexibility, strength, coordination and work simulation activities. She teaches a home exercise program and provides nutritional direction for weight loss and pain management. She assists in restoring a confidence in the pain patients ability to return to work.

Lauren Denenberg, PT, has 22-years of experience as a licensed Physical Therapist. She graduated from The University of Toronto. She has expertise in core stabilization for backs, and necks with an emphasis in functional re-training. She provides specialized exercises for individuals with chronic pain. She helps restore physical function using work-hardening and conditioning exercises to enable individuals to return to work.

Jesus Arroyo, QRR, is a Qualified Rehabilitation Representative. He provides counseling to help individuals prepare for employment. He is a bilingual counselor with an emphasis in assisting Hispanic workers to return to work at the end of the 8-week program. He employs vocational evaluations, counseling, job search assistance, writing resumes and cover letters for entering the job market. A goal of SPARCmed is to find employment for individuals who complete the program.

Robert Durazzo, MFT, is a bilingual, Marriage & Family Therapist, specializing in chronic pain rehabilitation. He was trained at the Stanford University Hospital Pain Service. Mr. Durazzo is a psychotherapist with a focus in behavioral medicine which includes cognitive/behavioral treatment, biofeedback, and treatment of anxiety, and depression.

Perry Blackmon, PhD, was trained in multidisciplinary care of chronic pain at Stanford University School of Medicine Pain Service. He holds a doctoral degree in Clinical Psychology and is a Licensed Marriage & Family Therapist. He has published numerous articles and teaches pain management. Dr. Blackmon specializes in treatment of emotional distress such as depression, anxiety and anger. Modalities he employs include cognitive/behavioral therapy, biofeedback, hypnosis and deep relaxation to help manage pain symptoms. He also provides family-counseling for interpersonal difficulties caused by chronic pain.
